Becoming a New Patient
W. R. “Ted” Kuball, D.D.S., P.L.L.C. is currently
accepting new patients.
In order to schedule an appointment, please call (269) 429-2555
and ask the staff for a new patient packet. You may receive this
packet via regular mail, e-mail, or by downloading the information
directly from this website. The new patient forms MUST be completed
prior to your first appointment and turned in to the office staff
at that time. Your original signature must be on all documents
where required.
If you are transferring your dental care from another primary
care dentist, please ensure that the "authorization for dental
records" has been completed and mailed to your current dentist
at least 2 weeks prior to your scheduled appointment. Failure
to do this may result in a delay of your medical record transfer
and disruption of your health care.
As a courteousy to those patients who may be awaiting a schedule
opening, failure of a new patient to show for his / her scheduled
appointment without notification will result in no further appointments
being scheduled.

Financial
Information
We believe that, clinically speaking, there is no reason to postpone
recommended treatment because postponement makes treatment more
complex, more expensive, less comfortable, and less certain. We
empower our patients to be proactive in decision-making concerning
their treatment recommendations and all financial planning.
Financial Arrangements
Our office will make every effort to make your dental care affordable.
A variety of payment arrangements is available through our Financial
Secretary. We will be happy to see that your insurance claims
are filed promptly. Read more in our Financial Information section.
Insurance
We will promptly file all the necessary dental forms to your insurance
provider to ensure that you will receive the maximum of benefits;
however, we can make no guarantee of any estimated coverage. Your
insurance policy is an agreement between you and your employer;
therefore, we ask that all patients be directly responsible for
all charges. Please know that we will do everything possible to
provide your insurance company with the necessary information
to ensure you receive the full benefits of your policy.
We are committed to your optimum dental health. We examine, diagnose,
and plan treatment for our patients comprehensively, taking into
account their expected outcomes. We will not allow insurance companies
to dictate or control how we provide treatment to you. Therefore,
treatment recommendations are made on what is best for your dental
health, not what is best for the insurance company.
Payment
Options
Please keep in mind that payment is expected at the time of service,
just like in any other business, whether or not insurance is involved.
We will work directly with you to offer a variety of payment options
that will allow you to plan effectively for optimal dental treatment.
Cash Discounts
We offer a Prepayment Courtesy Adjustment that allows for a 5%
discount to be given when cash or check is used to pay for treatment
at the time of service. This is our most economical option and
must be made by cash or check, not credit card. This discount
is available for restorative procedures only. The discount does
not include routine appointments scheduled with our hygiene staff.
Credit Cards
We are happy to accept Visa, MasterCard, American Express, Discover,
and all Banking Debit cards.
Dedicated Healthcare Credit Card
We offer the use of the Dental Fee Plan by Capitol One, a company
that offers financing for up to 60 months. Interest rates may
be interest-free for up to one year, after which typical credit
card rates would apply, or competitive fixed rates. The Dental
Fee Plan requires no initial downpayment, no prepayment penalty,
and covers dental fees from $1000 - $25,000. Our Treatment Facilitator
or Administrative staff can guide you through the process of obtaining
this form of credit, usually within twenty minutes of submitting
the application over the telephone. You may also obtain pre-approval
by visiting the Dental Fee Plan website.
